Our journey began in 2013, when Gjurgica Kjaeva founded the association. Personally touched by the challenges of living with pulmonary hypertension came together to form a structured platform for advocacy, support, and education. At that time, pulmonary hypertension was not widely recognized in medical registries and public health systems in North Macedonia. Through persistent grassroots efforts, awareness campaigns, and collaboration with medical professionals, APH Moment Plus played a key role in getting PH officially recorded in the national rare disease registry, ensuring that patients would no longer be invisible within the health system.Over the years, our association has become an active voice in the rare disease and patient advocacy community. We have organized public awareness events — from educational programs for doctors and students to participation in national activities like the Wizz Air Skopje Marathon, where our members run to raise visibility and funds for PH awareness and treatment access. As a member of the Pulmonary Hypertension Associations in Europe (PHA Europe), APH Moment Plus connects locally and internationally with organizations and experts committed to better care, research, and support for patients across borders. Our mission is simple yet profound: To improve awareness of pulmonary hypertension among patients, physicians, and the public. To advocate for timely diagnosis, effective treatment, and access to essential medications. To foster a supportive community where patients and families can share experiences, resources, and hope. Every initiative we undertake is driven by our belief that no one should face a rare disease alone  and that every additional moment of quality life is a moment worth fighting for.
